Before Your Visit
Steps Before Your Appointment
- Download the new patient packet PDF before your appointment.
- Gather your photo ID and any claim-related or referral documents.
- Bring prior records, imaging reports, or operative reports if available.
- Prepare a current medication list and any questions for the provider.
- Arrive early if you still need help completing any paperwork.
Arrival Recommendations
Please arrive 15–20 minutes before your scheduled appointment if you have not already completed all required paperwork.
If your forms are complete before arrival, check-in is usually faster and your visit can begin more efficiently.

New patients must download the printable PDF version, complete it before the appointment, and bring it with them to the office.
The PDF file must be uploaded to your website in the same folder as this page and named exactly new-patient-packet.pdf.

- Workers' compensation claim number, if assigned
- CA-1 or CA-2 form, if available
- CA-16 or other authorization documents, if available
- Accepted conditions, if known
- Prior treatment records related to the work injury
- Imaging, specialist reports, or operative reports related to the claim

Federal Workers
Important Information for Federal Workers
Emergence MAT works with federal workers and understands the importance of accurate documentation, timely evaluations, and organized medical records for work-related injuries.
If you are being seen for a federal workers' compensation case, please bring any OWCP-related paperwork, claim details, accepted conditions if known, and any records from prior providers related to the injury.

Telehealth Visits
Telehealth visits may be available for Florida residents who are already established patients, depending on the patient's needs and the nature of the visit.
